Johnannes was the son of Earl Johannes I Orsini of Kephalonia at Maria, a daughter of Nikephoros I Komnenos Doukas of Epirus and Maria Doukaina Laskarina. John's older brother Nikolaas Orsini had mastered Epirus in 1318 after the murder of their uncle Thomas I Komnenos Doukas. In 1323, John killed his brother and followed him, both in Kephalonia and in Eprus.
